- Congenital (hereditary) pendular nystagmus - described in Siamese and other colorpoint-derived breeds (which share the cs "Siamese" albinism allele with Ragdolls) due to misrouting of retinal ganglion cell axons at the optic chiasm; classically horizontal or pendular, present from a young age, non-progressive, and - critically - occurs in an otherwise neurologically normal cat with no ataxia or head tilt, which fits this presentation well.SciDirectWebvisionPMCMessybeastPMC
- Peripheral vestibular disease (otitis media/interna, nasopharyngeal/inflammatory polyp) - distinguishing feature is that it is almost always accompanied by head tilt, ipsilateral ataxia/circling, and often Horner's syndrome or facial nerve paresis; polyps in particular are common under 2 years of age but virtually always produce these additional signs, which this cat lacks.JFMSVCAJSAPMerckEveryCatACVS
- Idiopathic vestibular syndrome - acute, non-painful peripheral vestibular disorder, but by definition it presents with concurrent ataxia and head tilt, and though it can occur at any age it is more typical in older cats and again does not fit "no other signs".VCANCBI
- Central (neurologic) disease, including neurologic FIP - the single most important cannot-miss differential in a young cat, since FIP is the most common cause of neurologic dysfunction in cats under 2 years and can present with nystagmus (horizontal, vertical, or positional) as part of a vestibular/cerebellar syndrome; however, most reported cases still show accompanying ataxia, altered mentation, or postural reaction deficits, so a truly isolated nystagmus would be atypical for it.JFMSABCDJVIMUCD
- Toxoplasmosis, cryptococcosis, or aberrant parasite migration - reported causes of feline central vestibular signs in younger cats, but again these are described with concurrent neurologic deficits rather than nystagmus alone.JFMS
Mechanism
Congenital nystagmus in colorpoint cats is attributed to an abnormal ipsilateral-to-contralateral crossover ratio of optic nerve fibers at the chiasm, a pleiotropic effect of the tyrosinase-related mutation underlying the Siamese/colorpoint coat pattern; the resulting aberrant visual cortex mapping is thought to drive compensatory pendular eye movements and, in some individuals, concurrent strabismus.SciDirectWebvisionPMCPMC Because Ragdolls carry this same cs allele in their ancestry, the mechanism is directly applicable to this breed even though most published case descriptions use Siamese cats as the index population.WebvisionMessybeastPMC

Neurologic FIP deserves explicit exclusion in any young cat with new-onset nystagmus, even without other signs yet, since vestibular/cerebellar involvement can be an early or isolated finding before ataxia or mentation changes become obvious, and up to 30% of FIP cats show neurologic involvement.ABCDJVIMUCD
Narrowing test
A full otoscopic and oropharyngeal exam (to rule out polyp), fundic exam and menace/tracking assessment (congenital nystagmus is typically present since kittenhood and non-progressive, which historical review can confirm), and a complete vestibular/cranial nerve exam to look for any subtle head tilt, positional strabismus, or postural deficits are the key first steps before committing to an "incidental congenital" label.JFMSVCAJSAPEveryCat If any additional neurologic sign is found on repeat exam, MRI of the brain/tympanic bullae and CSF analysis (with FCoV RT-PCR) become indicated to rule out central causes including FIP.JFMSJVIMUCD

When to refer
